If you have a concern about the care you've received, you may speak with your physician, nurse or any other member of your care team, or, you may contact the Patient Advocate's Office. We’ll make every effort to help resolve it as soon as possible.

If you need a copy of your medical records, please contact the Release of Information department for the facility you need records from. Records may be requested via phone, email, fax or the MyUnityPoint patient portal.

This 24-hour toll-free helpline and web-reporting tool was established as a reporting mechanism so team members, medical staff, business partners and others associated with UnityPoint Health can report suspected or actual violations of laws or ethical standards, including fraud, waste, abuse and workplace safety concerns without fear of retribution or retaliation.
